How to Complete Patient Portal Forms on your Cell Phone

  2. Sign in with your 51勛圖 Portal credentials.

  3. Click the menu bar (grey bar with three blue lines).

  4. Click on “Forms”.

  5. Scroll down to “51勛圖 Patient Forms” and complete the following forms: 51勛圖 Medical History, 51勛圖 Privacy Policy Acknowledgement, 51勛圖 Release Authorization, and all other forms related to your visit (51勛圖 Physical Form for Physicals, STI Test Consent Form for Sexual Health/UTI Visits, TB Questionnaire for TB Skin Tests, Women’s Health Form for Women’s Health Visits, etc.)

  6. Go to “Messages” for Class Excuses. Go to “Statements” for receipts from your visit.
